im not saying that he is madara but in the nartuo world pepole can change the way they look.
for exp: natuto can turn into a girl. he also turned into sasuke and sai.. did you people forget about henge no jutsu

Danzou is using a henge no jutsu on one of his empty eye sockets to make it look like he still has one eye. As for being dead, the answer is simple - a jutsu.
