He didn't account for the human error, which in turn was an error since that meant he assumed things would go more perfect than it did.

I took it the opposite way. That Urahara couldn't have concieved of a plan going off perfectly without a hitch, such as Aizen's did in covering for his own ass then implicating Urahara in the whole thing. Also that he somewhat underestimated Aizen.
