1. madara lived long after his defeat.
2. he prly died due to a freak accident when he tried to merge his genes with hashiramas. he did manage to survive...

i think paulbee is right, izanami can be used in more ways than one, in itachis case he used a genjutsu to cast izanami. genjutsu has always been itachis strong point.
